Hypothyroid patients often get blamed for their weight. People assume it’s willpower, or that they’re simply eating too much. That’s not what’s going on. An underactive thyroid breaks the feedback loop between fat cells and the satiety centers in the hypothalamus, and the result is central leptin resistance.
The Satiety Loop: Leptin and the Hypothalamus
Leptin is made and released by adipose (fat) tissue, in direct proportion to how much body fat you carry. When the system works, high circulating leptin crosses the blood-brain barrier and binds to leptin receptors (LepRb) in the arcuate nucleus of the hypothalamus. Then three things happen:
- It stimulates POMC/CART neurons, which tells your brain you’re full and satisfied.
- It suppresses NPY/AgRP neurons, which shuts down hunger cravings and food-seeking behavior.
- It signals the thyroid to keep energy expenditure up.
How Low Thyroid Hormones Break the Leptin Sensor
Thyroid hormone — T3, specifically — is needed for the transcription and membrane density of leptin receptors in the hypothalamus. When thyroid hormone runs low, the signaling breaks down in two ways:
- Receptor downregulation: Without enough Free T3, hypothalamic leptin receptors lose sensitivity. Your fat stores can be saying “we have plenty of energy,” but the brain reads it as acute starvation.
- High fasting leptin: Because the brain never gets the message, fat cells pump out more and more leptin. Levels often exceed 40–60 ng/mL in hypothyroid women, and that constant flood desensitizes leptin receptors throughout the body.
The Starvation Trap: Why Cutting Calories Worsens Thyroid Weight Gain
Here’s the trap. A hypothyroid patient gains weight, then cuts calories hard — down to 1,000 kcal/day, say. Circulating leptin drops fast. The hypothalamus reads that drop as a famine and responds by downregulating hepatic 5′-deiodinase (DIO1) and converting available T4 into biologically inert Reverse T3. Metabolic rate falls by another 25%, and fat loss stalls completely.
Restoring Leptin Sensitivity: A Clinical Framework
Breaking leptin resistance starts with normalizing cellular Free T3 — aiming for the upper half of the range — while keeping post-meal insulin spikes steady. In clinical trials, eating 30 grams of clean protein within 45 minutes of waking helped anchor hypothalamic satiety signaling and blunt nighttime carbohydrate cravings.
